Tribal Youth Gift Giveaway

LCTHC's Human Services Department is hosting its Tribal Youth Gift Giveaway Drive-Up Event for local Tribal children ages 0-17 years old. Please see the attached flyer for more information. 

Attached you will also find the registration form that can be brought on the day of toy pick up, or turned in to the following locations:

• Lakeport

  • Behavioral Health Front Desk

  • Pediatrics Front Desk

  • Administration Front Desk

• Clearlake

  • Southshore Front Desk

LCTHC front desk staff will send completed forms to Legacy Court each week. Pre-registration ensures that you will receive toys. If you choose to fill out a form on-site the day of pick up, please be advised this is a first come-first served basis while supplies last.
